Diabetes Prevention Initiative

The County of Santa Clara Diabetes Prevention Initiative (DPI) works toward better health for all, ensuring that County residents with prediabetes are identified and connected to prevention education and healthy living resources in the community. This increases a person’s healthy lifespan and decreases the burden of disease by preventing individuals identified with prediabetes from progressing to type 2 diabetes.

The Diabetes Prevention Initiative Approach

The DPI takes a health equity approach, creating the conditions necessary to support health for all. The initiative focuses on improving health for folks experiencing socioeconomic disadvantages or historical injustices, including the large Medi-Cal population in the County of Santa Clara. The Diabetes Prevention Initiative is led by the County of Santa Clara Public Health Department and engages a wide range of stakeholders to take action on the following areas:

  • Raise pre-diabetes awareness
  • Establish prediabetes and diabetes screening and referral programs in the community and clinic settings
  • Increase coverage for and access to evidence-based diabetes prevention programs 

In 2015, a three-year DPI Strategic Plan was created through a countywide effort by public agencies and non-profit groups. The plan outlined goals that all stakeholders worked on together to support County of Santa Clara residents in preventing type 2 diabetes. Learn more about the first strategic plan and the Together We Can Prevent Type 2 Diabetes Campaign.
