Child Health and Disability Prevention (CHDP)

The County of Santa Clara Child Health and Disability Prevention (CHDP) Program focuses on the health and well- being of children and youth from families with low or moderate incomes. CHDP helps prevent health problems and find any problems early on so they can be treated. Program staff works with health care providers and child advocate agencies to make sure that eligible children receive quality health care. Staff also empowers families to understand their health care options and make decisions that are best for their family.

Health exams for ELIGIBLE children and youth

Children and youth are eligible if they are:

  • On Medi-Cal and 0 to 21 years old

  • Low to moderate income and 0 to 19 years old

    • Children and youth may be able to receive short-term Medi-Cal for up to 60 days through CHDP Gateway.

Regular health exams can:

  • Help children and youth stay healthy
  • Identify health problems early so a provider can refer the child/youth for treatment as needed. It is easier to correct a health problem if it is found and treated at an early age. Early care also can prevent or reduce serious problems for the child/youth later in life.

Types of CHDP exams:

  • Well-baby and well-child exams
  • Preschool/ Head Start exams
  • School exams
  • Sport or camp physicals
  • Teen physicals
     

All CHDP exams include:

  • A history of the child's health development
  • Head-to-toe physical checkup
  • Height, weight and growth check
  • Review of the child's nutrition
  • Hearing and vision screening
  • Oral health screening (does not replace dental exam)
  • Immunization (shots) if needed
  • Blood and urine tests
  • Tuberculosis (TB) screening
  • Answers to your questions and an explanation of the results of the health exam
     

If the exams show a need for more testing or treatment, it is important to follow the health provider’s recommendations.
