How to apply for Women, Infants, and Children (WIC)

Due to COVID-19, we are currently only able to sign people up for WIC via telephone. Please call 408-792-5101 to schedule an appointment by phone.

​What to Have Ready for Your First WIC Appointment:

• Proof of monthly household income

  • ​​​Examples: ​​Check stubs from the most recent month, records from unemployment or disability, military earning statement, letter from Cal-Works, or Medi-Cal card

• ​Proof of address

  • Examples: Utility bill or other mail, rental agreement, or California ID

• Identification for yourself and each person you want to enroll

  • ​Examples: Driver's license, California ID, Medi-Cal card, green card, Social Security card, birth or baptismal certificates, baby's crib card from hospital, immunization records or medical records

• Proof of pregnancy, if mom is pregnant
