The Positive Connections Program provides comprehensive case planning and coordination for those individuals who have been newly diagnosed with HIV or are not currently in care in Santa Clara County. Our goal is to promote better health outcomes by ensuring access to healthcare and supportive services.
We work together with community-based organizations and individuals to provide high quality care that is culturally and linguistically appropriate.
Some of the services we offer are:
- Access to primary health care
- Linkages to benefits counseling, mental health, food and housing assistance, drug and alcohol treatment, emergency financial assistance, dental care, legal services, and case management
- Vida y Salud, a support group for Spanish speaking clients
- Patient advocacy
- Partner counseling and referral through Partner Services
Partner Services
Partner Services is a confidential service that assists individuals with an STI or HIV to notify partners of a possible exposure and provide free HIV testing. Public Health staff work with individuals to provide guidance and support through the notification process, and help empower the individual to notify their partner(s). Individuals also have the opportunity to remain anonymous, in which case Public Health staff can make the notification. With this option, the notification will be completely anonymous and there will not be any identifying information revealed about where or when the exposure occurred or by whom.
New Ryan White Program
Effective July 1, 2024, the County of Santa Clara’s Public Health Department, Sexual Health and Harm Reduction Program will be providing robust case management services to Santa Clara County residents with low income who are living with HIV.
Some of the services offered will include:
- Screening, enrollment, and re-enrollment in the Ryan White Program and AIDS Drug Assistance Program (ADAP)
- Medical and non-medical case management services, including comprehensive assessments and service plans
- Medical transportation services and emergency financial assistance
- Ongoing support in maintaining medical care, care coordination, and treatment adherence counseling
- Linkages to primary health care, mental health, food and housing assistance, drug and alcohol treatment, dental care, legal services, and support groups
Frequently Asked Questions (FAQs)
No. Your current enrollment will remain active until your next re-enrollment date (also your birthdate). If your re-enrollment date is before July 1, 2024, you will work with your current case manager at The Health Trust to complete the required paperwork. Anyone with a re-enrollment date after July 1, 2024 will complete their re-enrollment with a case manager at Public Health. Your new case manager will contact you before your re-enrollment date using the contact information you provided to The Health Trust. Your information will automatically transfer over from The Health Trust.
Starting on July 1, Public Health will accept appointment requests. We will update this page once a phone number and email are available. A Public Health case manager will also reach out to you before your re-enrollment date.
Services will be provided at 976 Lenzen Avenue, Suite 1500 in San Jose which is 0.4 miles from The Health Trust offices. Appointments are required for most services.
Yes, to wind down services The Health Trust Case Management team will not be seeing clients in the last two weeks of June. The Health Trust will be working with clients to complete all June re-enrollments before their closure. While the official launch of the Ryan White Program in Public Health is not until July 1, 2024, clients with an urgent need between June 15 and July 1 should contact Public Health. We will update this page before June 15 with the email address and phone number for the program.
There are no changes to current eligibility requirements.
To be eligible for the Ryan White Program, you must have a diagnosis of HIV or AIDS, live in Santa Clara County, and meet all financial qualifications.
Through June 14,2024, please reach out to The Health Trust at [email protected]. After June 14, 2024, new clients will be enrolled by Public Health. More information to come.
Starting July 1, food services previously received through The Jerry Larson Food Basket will be provided by Loaves and Fishes. Clients currently accessing food services will not need to re-establish eligibility to begin receiving the same services with the new provider. Beginning July 2024, Safeway Food Vouchers will be distributed by the Ryan White Program at the Public Health Department as the new case management organization. More information will be posted here as soon as it is available.
Clients receiving housing services funded by the Ryan White Program, also referred to as Housing Program Plus or HPP, will be provided by another community organization. Those plans are currently being finalized and updates will be posted here as soon as they are available.
